Is infill a one time thing or do I have to keep applying it over time? If so, how often? I'm assuming with more traffic the sand will fall out slowly.

It all depends. In many cases, you don’t have to add infills often if your artificial grass lawn is on a flat surface with out much of traffic or rains. Areas with a slope or with a lot rains, from time to time, infills will be washed away or slip away in which situation you will need to add infills. It is not necessarily a good idea to set the frequencies of adding infills, rather doing so only when it is needed.
